Fixed chained block break rollbacks

Fixed handling door breaks

Fixed tracking vine breaks

Cactus breaking on adjacent block place is now logged as a block_break if both block_break + block_place logging are enabled

End portal creation is now logged as a block_place if both block_place + player_interact logging are enabled

Disabling your tool will do three things at the moment. It will 1. disable your tool 2. print an error saying "off" or "disable" isn't a valid material then 3. set your tool to air. Coincidentally, this will effectively disable the tool, just in a very round-about way. This will be fixed in 1.1.3.
