Added optional penalties for logging out after taking damage. Checks if you were damaged within a certain time (configurable) of logging out, and if so, deals damage equal to a percentage (also configurable) of your health.

Changed how HeroScoreboard hooks Heroes.

Fixed pvp level range not working.

* Weapon, Skill, and Nemesis dont' seem to be stored properly
