⚙ Optimizations
• Fast-reject NPCs with no icon config before any per-tick work.
• Write icon transforms in place, only when changed (no per-tick allocs).
• Cache quest-state from Core across ticks instead of querying every tick.
• Stagger that cache refresh per player to spread load on busy servers.
• Throttle orphan-icon cleanup to ~once per second.
