Tracker freeze fix

Fixed

  • Tracker no longer freezes — stale cache auto-clears after 5 seconds if scanning stops, so phantom markers disappear instead of staying forever
  • Watchdog logging — warns in server log if no successful scan for 10+ seconds
  • Silent exceptions now logged — world.execute failures and NPC processing errors are no longer swallowed silently
