There is no item level or enchant status showing on my off-hand weapon. All other slots are working fine.

Sounds like u changed the lua for the Neck update and disabled the Offhandslot. Change it to this and your neck and your offhand will be shown:

SOLUTION:

Open OilvlPlayer.lua

Go to Line 257

Replace Line 257-272 with:
