The author includes 3 GREAT versions that you should explore! NOTE: The author did not update 0.8.8 to 0.8.9 in the res_mods folder. If you copy the whole tree to install, change that. Otherwise, simply place the scaleform folder of your favorite version in your gui folder.
To get one exactly like mine (pictured), use my CDPData file with Version 1 download from Frost Zone. Or, Edit it to your satisfaction, using the attached .jpeg as a guide, with >>Notebook<<. Do not edit in Word.
